	MANDATORY FORMS:  Emergency Contact and Medical Information: Please print and submit one form for each of your children. Forms should be given to Nurse Paige. Family Information Form: All families need to complete this online form.   Co-op Form/Skills Inventory Form: Please submit this online form immediately so we can begin to match you with co-op opportunities that fit your skills. Health Appraisal Form: Mandatory for new students and returning students whose appraisals are due for updating. Please note that the school nurse should be made aware of significant health concerns such as food allergies, asthma, diabetes, epilepsy, etc. prior to the start of school. Please print and return to Nurse Paige.   OPTIONAL: Adopt-A-Book Program Form: Visit the Adopt-A-Book page to find paperwork needed to support the WMS library through this program.  Advance Childcare Registration Form: Use this form to register for childcare on days when classes are not held at WMS. Pennsylvania Residents' Form: For Pennsylvania Residents living within 15 miles of WMS - if your child is using a school bus, please complete the Pennsylvania School Bus Riders Form and return to your public school district.
